چکیده

In this issue of the Circulation: Arrhythmia and Electrophysiology, Kurotobi et al1 determined the number of arrhythmogenic foci in atria provocable with isoproterenol infusion in patients with paroxysmal, persistent, and permanent atrial fibrillation (AF). The results show that persistent AF has more foci than paroxysmal AF. However, when AF persists for 12 months, the number of foci were reduced. The authors suggest that the increased number of foci probably is involved in the development of persistent AF. However, because this association is not observed for AF that persists for 12 months, the importance of these foci in AF maintenance remains unclear.

Multiple arrhythmogenic foci associated with the development of perpetuation of atrial fibrillation.

BACKGROUND The presence of multiple arrhythmogenic sources may be associated with the perpetuation of atrial fibrillation (AF). In this study, we investigated the hypothesis that multiple foci might be involved in the development of AF persistency. METHODS AND RESULTS Two hundred fourteen consecutive patients with AF undergoing catheter ablation were enrolled in this study. The location of th...

Simultaneous noncontact mapping of left atrium in patients with paroxysmal atrial fibrillation.

BACKGROUND Catheter ablation of pulmonary vein ectopic foci is a potentially curative treatment strategy for patients with atrial fibrillation. However, identification of arrhythmogenic pulmonary veins with conventional mapping is difficult, especially in patients with rare focal activity, multiple active foci, or extrapulmonary foci. The present study was designed to investigate use of simulta...

Protective effects of simvastatin on atrioventricular node during simulated experimental atrial fibrillation in vitro

Introduction: Recent evidence has indicated that statins can reduce the incidence of both supraventricular and ventricular arrhythmias with various mechanisms. The primary goal of the present study was to determine direct protective role of simvastatin in modifying concealed conduction and the zone of concealment in a simulated model of atrial fibrillation (AF) in an isolated atrioventricula...

Electrophysiological end point for catheter ablation of atrial fibrillation initiated from multiple pulmonary venous foci.

BACKGROUND The end point for catheter ablation of pulmonary vein (PV) foci initiating atrial fibrillation (AF) has not been determined. METHODS AND RESULTS Ninety patients underwent mapping during spontaneous or induced ectopy and/or AF initiation. Ostial PV ablation was performed by use of angiograms to precisely define targeted sites. Success defined by elimination of AF without drugs was c...

Large Circular Ring Catheter Ablation Versus Anatomically Guided Ablation of Atrial Fibrillation: Back to the Future for Successful Catheter Ablation of Atrial Fibrillation?

Radiofrequency catheter ablation of atrial fibrillation (AF) is an effective rhythm control strategy, and its efficacy has been proved to be superior to anti-arrhythmic drugs.1-3) Recently, the European Society of Cardiology’s 2010 revised guidelines for AF management suggested catheter ablation as the firstline treatment in patients with paroxysmal lone AF.4) When Haissaguerre et al.5) initial...
